I don’t know that I have ever seen patternwork on leather. Decorative stitching, sure. Straight up embroidery, no.

There are special needles for leatherwork; sometimes a small awl is used to make holes for the thread. The thread has to be heavier, obviously, for leather than fabric.
So you punch a series of holes and it becomes a sort of counted cross stitch on leather?
A lot depends on how thick the leather is. Needles made for leather can do other stitches; sometimes the work is done on very thin leather which is bonded to a heaver leather suitable for belts and such like. You can do curves, which you can’t do on counted cross stitch. More like crewel embroidery, where the pattern is prepared on the fabric before you begin to stitch.
